Patients
The study was conducted between January 1992 and May 1993 at the Medical ICU of the Regional Medical Center and The University of Tennessee Medical Center, Memphis. The protocol was approved by The University of Tennessee institutional review board, and informed consent was obtained before patients were entered into the study. We prospectively studied 27 consecutive patients who developed ARDS. Patients entered the study within 24 h of developing ARDS.

Clinical Variables at Entrance
Twenty-seven patients entered the study: 16 were male and 11 were female. The mean (±SD) age was 47±13 years. Mean APACHE II score on hospital admission was 24±9, and predicted mortality was 43±25%. Mean LIS was 2.85±0.6 with all but six patients having an LIS greater than 2.5. ARDS was caused by a direct lung injury in 18 patients, including 16 with pneumonia and 2 with chemical aspiration. Intercellular communication through cytokine networking is essential for tissue homeostasis and provides a defense against infectious and noninfectious agents. Conclusions
By measuring BAL and plasma IC levels at the onset of ARDS and throughout the disease, we have provided data indicating that the degree of initial pulmonary inflammatory response sets the pace for the evolution of ARDS and may ultimately determine patient outcome.
