Study Population
Nineteen patients with squamous cell lung cancer were included. The diagnosis was confirmed by bronchoscopic biopsy specimens in all cases. Sixteen patients were men and 3 were women. Mean age was 65 years (range, 50 to 75 years). None of the patients had evidence of distant metastasis by bone and liver scans. Criteria for entry were stage IIIA or IIIB according to a new international staging system17 and performance status was no less than 70 percent by the scale of Karnofsky and Burchenal.18
Changes in the Number and Proportion of Lymphocyte Subsets by RT
The number of lymphocytes, CD3- and CD4-positive lymphocytes significantly decreased in peripheral blood of the patients with lung cancer before RT when compared with those of normal control subjects. However, the numbers of CD8-positive lymphocytes, NK cells, and B cells were not different from those of control group. Discussion
In this study, we confirmed that the patients with lung cancer had decreased numbers of circulating lymphocytes and RT accentuated more cancer-associated lymphopenia. Previous studies of RT-induced immune alteration in patients with bronchogenic carcinoma and other malignant diseases20, 21, 22, 23, 24 also showed that RT led to a decrease in the number of lymphocytes and lymphocyte subsets. These results agree with previous reports showing an increased frequency of activated T cells during RT (59). In contrast, however, at a later point in the transfused group, downregulation of the IL-2 receptor with a further increase in HLA-DR expression on CD3+ T cells was consistently detected.
