Sex Differences in Normal Electrophysiology: Clinical Aspects and Animal Models
By accepted convention, the term “sex” refers to biologically determined differences, whereas “gender” refers to socially/culturally-derived distinctions between male and female individuals. We follow these usages in our presentation. Men and women differ in the normal electrophysiology of their specialized conduction system and working myocardium (Fig. 1).
Supraventricular tachycardias
The sex dependence of paroxysmal supraventricular tachycardias (SVTs) varies with arrhythmia type (Fig. 3).31 Women are more likely than men to have AVN re-entrant tachycardia (AVNRT) and focal atrial tachycardia.32 Men have double the incidence of accessory pathway tachycardias as well as asymptomatic pre-excitation.32, 33 Sex affects pathway location, women having more right-sided pathways.33, 34 The small risk of ventricular fibrillation (VF) in patients with ventricular pre-excitation seems
Drug-induced long QT syndrome and TdP
The most common molecular mechanism of drug-induced long QT syndrome (LQTS) is blockade of hERG (Kv11.1), the α-subunit of the rapid delayed rectifier K+ current (IKr).68 Women have greater risk of drug-induced LQTS/TdP.69, 70 The causes are multifactorial. First, women show lower levels of expression of HERG and other K+-channel subunits like minK (β-subunit of IKs) and thus smaller repolarization reserve.28 Second, women show estrogen-dependent upregulation of Na+-Ca2+ exchanger and ICaL,
Coronary artery disease and heart failure
SCD rates in women are less than half those in men.94 Although age dependence is qualitatively similar, comparable SCD incidence occurs 10 years later in women. Conclusions
Although we have a reasonable understanding of mechanisms underlying sex differences in rare channelopathies and arrhythmias caused by repolarization abnormalities, we still have much to learn about the nature and basis of sex-related differences in more common, yet more complex, arrhythmias. Therapeutic decisions are modulated by sex, but how much of the modulation is evidence based vs a reflection of bias is unclear. Much more must be known before sex can be used appropriately to base
