The first external validation data set came from 7,485 hospitalized patients with acute STEMI from January 2012 to August 2019 in China. The second external validation data set came from 129,042 hospitalized patients with acute STEMI from January 2016 to December 2018 in the United States. We used two-class multiple logistic regression analysis to analyze the risk factors of in-hospital mortality in the development data set. We developed a diagnostic model of in-hospital mortality and constructed a nomogram. We evaluated the predictive performance of the diagnostic model in the two validation data set by examining the measures of discrimination, calibration, and decision curve analysis (DCA).Results In the development data set, 61 of the 2,183 participants (2.8%) experienced in-hospital mortality. The strongest predictors of in-hospital mortality were advanced age and cardiogenic shock. Logistic regression analysis showed the difference between the two groups with and without in-hospital mortality in age (odds ratio [OR] 1.0755, 95% CI 1.046016-1.105814; P <.001), and cardiogenic shock (OR 16.48628, 95% CI 9.435111-28.80702; P <.001). We had developed a diagnostic model of in-hospital mortality. The area under the receiver operating characteristic curve (AUC) was 0.8977 (SD 0.0198, 95% CI 0.85894 - 0.93636). We constructed a nomogram based on age and cardiogenic shock. In-hospital mortality occurred in 127 of 7,485 participants (1.7%) in the first validation data set. The AUC was 0.9179(SD 0.0134, 95% CI 0.89158- 0.94424).In-hospital mortality occurred in 14,064 of 129,042 participants (10.9%) in the second validation data set. The AUC was 0.7679(SD 0.0023, 95% CI 0.76346-0.77228).Conclusion The strongest predictors of in-hospital mortality were advanced age and cardiogenic shock. We had developed and externally validated a diagnostic model of in-hospital mortality in acute STEMI patients.
